Eligible pilots receive their medical certificate at the time of the exam. Dr. Shockey will discuss any next steps if additional FAA review is needed.
Your Examiner
Dr. David Shockey grew up in Mesa, Arizona — a Mountain View graduate who earned dual bachelor's degrees in Biological Science and Psychology from Arizona State University, summa cum laude. He received his M.D. from UT Health San Antonio, graduating first in his class, and completed his Emergency Medicine residency at the University of Arizona.
He is a certified Senior Aviation Medical Examiner, designated by the FAA to perform Class I, II, and III examinations. A private pilot himself, he understands what it means to sit on the other side of the exam table — and he approaches every certification with that perspective in mind.
Beyond aviation medicine, Dr. Shockey practices emergency medicine in Scottsdale, serves as a ringside physician, and founded the Desert Premier Wellness primary care program. A lifelong explorer, he has traveled to all seven continents and completed expeditions to Everest Base Camp, Kilimanjaro, and Antarctica. When not flying or working, he enjoys hiking, pickleball, and basketball.

A $50 reservation fee is collected at the time of online booking to secure your appointment. This fee is credited in full toward your visit balance — it is not an additional charge. Cancellations made more than 48 hours before your appointment receive a full refund to the original payment method.
Yes — prior to your appointment you'll need to complete your MedXPress application at medxpress.faa.gov. This is your official FAA medical application. Once submitted, the FAA provides a 12-digit confirmation number — please save it, as Desert Premier Aviation will need it before check-in. Complete MedXPress before your appointment and save your 12-digit confirmation number.
Bring your MedXPress confirmation number, a government-issued photo ID, your glasses or contacts if you use them, and any Special Issuance authorization (SODA) if applicable. If you have ongoing medical conditions, bring recent records from your treating physician plus a complete medication list with dosages. Please also arrive with a full bladder — a urine sample is required as part of the examination.
A Senior Aviation Medical Examiner is authorized to perform all three classes of FAA medical examinations, including First Class exams for airline transport pilots. Dr. Shockey's Senior AME designation means he is authorized to perform all three classes of examinations, including First Class for airline transport pilots.
A deferral means the FAA needs additional information before issuing your certificate — it is not a denial. Dr. Shockey will explain what additional documentation or testing is needed. Many deferrals resolve successfully. We recommend consulting an aviation attorney if you have a complex medical history.
Walk-ins are not available for FAA medical exams — an appointment is required. Same-week appointments are often available. Book online or call 480-649-9000. Pilots employed by select airlines may have access to additional appointment availability when clinic capacity allows.
The Preflight Physical Consult is a dedicated $150 in-person appointment with Dr. Shockey designed for pilots who want to discuss medical concerns before scheduling their official FAA exam. It is appropriate for pilots with medication questions, prior medical history concerns, previous FAA issues, or any situation where you want a knowledgeable physician's perspective before committing to the exam. This consultation is not an FAA medical exam and does not produce a medical certificate or guarantee any certification outcome.
For pilots under 40, a First Class medical is valid 12 months for ATP privileges. For pilots 40 and older, it is valid 6 months for ATP privileges, but remains valid 12 months for commercial privileges. Third Class medicals are valid 60 months for pilots under 40, and 24 months for pilots 40 and older.
